Responsibilities- Patient Care Coordination: Implement and monitor patient care plans, ensure continuity of care, and facilitate effective communication among healthcare team members.
- Clinical Expertise: Provide direct nursing care, assess patient needs, and develop individualized care plans that promote optimal outcomes.
- Coordination and Communication: Serve as a primary coordinator for all disciplines, ensuring seamless care transitions and maintaining open lines of communication with patients, families, and healthcare teams.
- Order Management: Note and carry out physician and nursing orders, ensuring timely and accurate execution.
- Discharge Planning: Assess and coordinate patient discharge planning needs with the healthcare team, ensuring a smooth transition to post-acute care settings.
- Competitive Salary: $84,000 - $110,000 per year, based on experience
