This position is responsible for providing direct nursing care and implementing patient care plans in accordance with established policies and protocols. The ideal candidate will have excellent communication skills and be able to coordinate with various healthcare disciplines to deliver high-quality patient care.

Responsibilities
  • Patient Care Coordination: Implement and monitor patient care plans, ensure continuity of care, and facilitate effective communication among healthcare team members.
  • Clinical Expertise: Provide direct nursing care, assess patient needs, and develop individualized care plans that promote optimal outcomes.
  • Coordination and Communication: Serve as a primary coordinator for all disciplines, ensuring seamless care transitions and maintaining open lines of communication with patients, families, and healthcare teams.
  • Order Management: Note and carry out physician and nursing orders, ensuring timely and accurate execution.
  • Discharge Planning: Assess and coordinate patient discharge planning needs with the healthcare team, ensuring a smooth transition to post-acute care settings.
