Job Summary

We are seeking a highly skilled and compassionate Inpatient Case Manager to join our team at LifeBridge Health. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions between hospital stays and post-discharge services.

Responsibilities:
  • Perform comprehensive assessments to identify patients' needs and develop personalized care plans.
  • Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes.
  • Coordinate discharge planning and arrange for necessary resources, including home health services, rehabilitation programs, and community-based support.
  • Maintain accurate and timely records of patient interactions and care coordination activities.
  • Stay up-to-date on changing regulatory requirements, best practices, and industry trends to provide exceptional care and meet quality standards.
Requirements:
  • Registered Nurse (RN) license in Maryland or intent to obtain MD licensure if out of state.
  • 3-5 years of experience in case management, nursing, or a related field.
  • Case Management Certification preferred.
  • Basic professional knowledge, equivalent to a Bachelor's degree, with working knowledge of theory and practice within a specialized field.
Salary and Benefits:

The estimated salary for this role is $40.59-$63.95 per hour, based on experience. Our comprehensive benefits package includes medical, dental, and vision coverage, 401(k) matching, and generous paid time off.
