Job Summary

We're seeking an experienced RN with a passion for case management to join our team. As a Case Manager II, you'll assess patient needs, create comprehensive care plans, and facilitate multi-disciplinary collaboration to ensure optimal patient outcomes.

Responsibilities
  1. Patient Assessment and Care Planning: Assess individual patients' needs for care coordination, discharge planning, and utilization management services.
  2. Care Coordination: Facilitate early referral to high-risk case management, physical therapy, occupational therapy, social work, risk management, patient advocacy, post-acute services, and quality management.
  3. Multidisciplinary Collaboration: Collaborate with colleague case managers in providing coordination of care and develop treatment plans, including contingency plans.
Qualifications
  • Licensure as a Registered Nurse (RN) in the state of Maryland or eligibility for licensure is required. A health-related Master's degree and certification as a case manager are both preferred.
  • One year of experience in case management and knowledge of payer mechanisms and clinical utilization management are preferred. Two years of experience in acute care are required, with four years of clinical nursing experience preferred.
Compensation and Benefits
