MDS Nurse LPN Coordinator Role

Job Description:
The LPN MDS Coordinator plays a crucial role in the Resident Assessment Process by completing pertinent items on the Minimum Data Set (MDS) and accompanying Care Area Assessments (CAAs). This ensures accurate patient care and reimbursement for services provided.
The successful candidate will direct the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, following guidelines for completion, computer encoding, and electronic State transmission of the MDS. They will also participate in developing individualized resident Care Plans, identifying needs, and projected outcomes, as required by Federal and State regulations.
In addition, they will train new team members on clinical content and electronic completion of applicable MDS items.
