Nurse Care Coordinator Role

Dartmouth Health seeks a skilled Registered Nurse to serve as a Nurse Care Coordinator in our Medicine Service. This role is an excellent opportunity for a dedicated professional to make a meaningful impact in the lives of patients and their families.
As a Nurse Care Coordinator, you will play a critical role in ensuring that patients receive comprehensive and coordinated care throughout their healthcare journey. This position offers a competitive salary of $85,000 - $110,000 per year, depending on experience. The ideal candidate will have a strong background in nursing, with at least 3 years of clinical experience. A Bachelor's degree in Nursing is also required, and recent acute care hospital experience is highly preferred.
In this role, you will work closely with interdisciplinary teams to assess patient needs, identify areas for improvement, and implement evidence-based practices. You will also be responsible for communicating effectively with patients, families, and healthcare professionals to ensure seamless care transitions and optimal outcomes.
