Job SummaryThe successful candidate will be responsible for assessing patients' physical, emotional, and socio-economic needs, providing direct skilled nursing care, and communicating effectively with healthcare practitioners. You will also participate in quality assurance and performance improvement initiatives, and work collaboratively with multidisciplinary teams to deliver high-quality patient care.
Responsibilities- Assess patients' physical, emotional, and socio-economic needs through thorough evaluations and observations.
- Develop and implement individualized plans of care, working closely with patients, families, and healthcare professionals.
- Provide direct skilled nursing care, adhering to physician orders and documenting care accurately.
- Counsel and educate patients, families, and caregivers on various aspects of care and health management.
- Participate in quality assurance and performance improvement initiatives, contributing to the development of policies and procedures.
- Bachelor's degree in Nursing or equivalent experience.
- Strong research skills and effective interpersonal communication abilities.
- Demonstrated knowledge of systems integration and ability to adapt to new technologies.
- Minimum one year of experience in a qualified home health agency.
We offer a competitive salary range of $32.64 - $65.01 per hour, based on experience and qualifications.
