Client Care Coordinator Job Summary

Homes for the Homeless is seeking a skilled Client Care Coordinator to provide mental health and related services for homeless families. The ideal candidate will enhance existing services by conducting thorough assessments of families' long-term goals and collaborating with case management staff to set achievable goals and establish linkages within the community.

Key Responsibilities:

  • Provide mental health and related services for homeless families
  • Conduct thorough assessments of families' long-term goals
  • Collaborate with case management staff to set achievable goals and establish linkages within the community
  • Monitor and document client progress toward service plan goals
  • Ensure that families are receiving full support in shelter and refer families for counseling or other services as needed
  • Support the Family Services department by working alongside the Case Manager and supporting staff when dealing with challenging clients
  • Improve linkages to mental health, education, and community-based services
  • Strengthen the overall permanency outcomes for families with children in shelter
  • Manage record keeping, data collection, and evaluation on the effectiveness of services for families
  • Assist with unit inspections when needed
  • Support the Family Services department by conducting trainings and promoting best practices for Family Services staff

Requirements:

  • Masters degree in Social Work or Mental Health Counseling
  • Familiarity with substance use disorders and addiction treatment services, domestic violence, child abuse and maltreatment, trauma, and mental health conditions
  • Sensitivity and awareness of working with homeless and vulnerable populations
  • Bilingual is a plus
