Job Title: Nurse Specialist - Long Term Care

About the Role: We are seeking a skilled and compassionate Nurse Specialist to join our team in providing exceptional care to patients with complex needs in a long-term care setting.

Responsibilities: Provide high-quality patient care, develop and implement individualized care plans, and collaborate with interdisciplinary teams to ensure seamless transitions of care.

Requirements: Current RN licensure, 2+ years of experience in gerontology or long-term care, excellent communication and interpersonal skills.
