The Home Care Registered Nurse is tasked with the comprehensive supervision, evaluation, planning, and implementation of nursing care provided to participants. While the primary nurse may assign certain direct care, data collection, and nursing assessment duties to other nursing staff and assistants, they maintain accountability for ensuring that all interventions align with the participant's care plan and the directives of their healthcare providers. Care may be delivered in various settings, including community health centers or participants' residences.
Key Responsibilities:
- Conduct thorough patient assessments by gathering medical history, performing relevant tests, and collaborating with other members of the interdisciplinary team.
- Prepare and present patients for prompt evaluation and treatment by healthcare providers.
- Coordinate or directly execute care plan interventions, testing, treatments, or other provider orders for a designated group of participants.
- Supervise delegated tasks to ensure their completion, monitor outcomes, and adjust care plans as necessary.
- Administer medications, tests, and treatments safely and effectively as prescribed. Manage medication refills with appropriate provider authorization.
- Educate patients and their families on healthcare regimens, providing clear explanations regarding conditions, wellness, tests, results, medications, diet, exercise, and other components of the care plan.
- Bachelor's degree in nursing is preferred.
- Active nursing license in Massachusetts.
- Current BLS certification is required.
- At least 2 years of experience in ambulatory care, home health, hospital, or sub-acute settings is preferred; equivalent training or related experience will be considered.
- A minimum of one year of experience working with elderly or frail populations.
