Key Responsibilities- Provide basic nursing services to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' needs and develop individualized care plans to meet those needs.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Maintain accurate and detailed records of patient interactions, including progress notes and medication administration records.
- Participate in ongoing education and training to stay up-to-date on the latest home health care best practices and regulations.
- A current Massachusetts LPN license.
- 1-2 years of experience as an LPN, preferably in home health care or a related field.
- 1-2 years of recent Medicare-certified home care experience or related health field.
- Knowledge of Medicare Conditions of Participation and OASIS documentation.
- A valid driver's license and reliable transportation.
- Knowledge of home health equipment and supplies.
- Experience working with diverse patient populations, including those with complex medical needs.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- A competitive hourly rate of $25.00 or more, depending on experience.

- A flexible schedule, with opportunities to work Monday through Friday and occasional weekends.
