Job Title: Behavioral Health CBRF Caregiver ManagerJob Summary:

Trinity Care is seeking a highly skilled and compassionate Behavioral Health CBRF Caregiver Manager to join our team. As a key member of our care team, you will be responsible for providing exceptional care and support to our residents with behavioral health needs.

Key Responsibilities:
  • Provide leadership and oversight to caregiver staff to ensure high-quality care and services are delivered to residents.
  • Develop and implement individualized care plans that meet the unique needs of each resident.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
  • Ensure compliance with regulatory requirements and company policies.
Requirements:
  • C.B.R.F. Certification
  • Minimum of 18 months of experience in home care service delivery or coordination.
  • Proven ability to work with clients with various behavioral/mental health diagnoses.
  • Strong leadership and organizational skills.
  • Excellent interpersonal, written, and verbal communication skills.
