LPN Patient Service Coordinator

Job Summary
We are seeking a highly skilled LPN Patient Service Coordinator to join our team at Amedisys. In this role, you will be responsible for scheduling home visits for our field staff and assisting with the coordination of patient care.
Key Responsibilities
- Ensure timely scheduling of patient assessment visits, including Oasis visits.
- Prepare weekly clinician schedules and review them daily to determine capacity opportunities and ensure productivity standards are met.
- Assign and schedule clinicians in the most efficient geography, maximizing clinician efficiency and optimizing capacity.
- Address missed visits by the end of the current week or refer them to the Clinical Manager for resolution.
- Collaborate with field clinicians to ensure all patient visits are scheduled and completed as ordered and within the care center.
- Manage calls from patients and field staff related to scheduling issues and refer clinical and performance-related issues to the Clinical Manager.
- Monitor hospitalized patients and communicate with the team to ensure follow-up as necessary.
- Complete requested schedules, ensuring all orders and services are included as required.
- Support the Clinical Manager and work collaboratively to ensure timely communication with patients, clinicians, referral sources, and other office personnel.
- Run applicable reports and respond to workflow, taking appropriate actions.
- Assist with internal or external transfer of patients between care centers and/or Hospice services.
- May provide direct patient care as necessary and only in times of an emergency, if licensed and meets all criteria to be a field clinician.
- May be responsible for maintaining supply closets and serving as a liaison with the field for patient supply needs.
- In the absence of an Intake Coordinator, may be responsible for the referral and intake process within the care center.
- May obtain verbal orders for patients and document as applicable.
