In the role of Clinical Portfolio Manager, you will operate under the guidance of the Market Leader (ML) to provide support to local clinical leaders and resources across various agencies within the designated market or region. This position is crucial in nurturing leaders who can significantly enhance the capabilities and outcomes of agency DCS/DORs.

The Clinical Portfolio Manager is dedicated to fostering leadership development, ensuring that leaders and clusters within the market are well-equipped to deliver exceptional quality of care in a cost-effective manner.

Working closely with the Market Leader, the Clinical Portfolio Manager collaborates to cultivate a company environment that offers transformative services across the four cornerstones: culture, community, clinical/compliance, and finance.


ESSENTIAL FUNCTIONS

To enable individuals with disabilities to perform essential functions, reasonable accommodations may be made.

Support local leadership in establishing and executing processes to monitor and assess activities related to clinical operations, education, and performance enhancement.

Provide expertise in orientation, new hire competency systems, annual skills assessments, and ongoing educational program development aimed at improving staff retention.

Oversee the recruitment, orientation, onboarding, and continuous leadership development of key clinical leaders within the market's agencies, in partnership with the agency's Executive Director, with the ultimate aim of cultivating clinical leaders into C-level executives.

Collaborate with resources to address field needs in system, process, and best practice design, education, and dissemination.

MINIMUM EDUCATION AND/OR LICENSURE

Licensed Registered Nurse or related licensed discipline (e.g., PT/OT/ST, etc.).


MINIMUM EXPERIENCE

A minimum of five years of experience in Home Health and/or Hospice settings, with prior experience in agency leadership.


REQUIRED SKILLS, ABILITIES AND/OR LICENSURE

Comprehensive understanding of the legal and regulatory framework governing the home health/hospice sector.

Ability to collaborate with all management levels on various aspects of clinical operations.

Strong verbal and written communication skills to articulate complex and/or confidential information.

Excellent time management and organizational skills to manage multiple projects, prioritize workload, and plan effectively for agency leaders to meet objectives.

Proven ability to interact positively with employees and anticipate and identify challenges, employing sound judgment and fact-based analysis to devise effective solutions.

Proficiency in software applications, including Microsoft Office and web-based tools.
