Job Title: RN Care Navigator

As a RN Care Navigator at Providence, you will play a vital role in managing patient populations with chronic illness and/or multiple co-morbidities. You will work in coordination with multiple PCPs and serve as the central link to an identified population of patients.

Key Responsibilities:
  • Manage patient populations with chronic illness and/or multiple co-morbidities
  • Work in coordination with multiple PCPs
  • Serve as the central link to an identified population of patients
Requirements:
  • Graduation from an accredited school of Nursing
  • Washington Registered Nurse License upon hire
  • National Provider BLS - American Heart Association upon hire
  • 2 years - Work experience in an RN role
Preferred Qualifications:
  • Bachelor's Degree - Nursing
  • Certification of diabetes education (CDE) upon hire
  • Experience managing complex patients in an ambulatory setting
  • Medical Home Model experience
