About the Role

Providence is seeking a highly skilled and experienced Home Health Clinical Manager to join our team. As a key member of our Home Health Clinical Team, you will be responsible for coordinating and managing the delivery of high-quality, patient-centered care to our patients in their homes.

Key Responsibilities:

  • Coordinate and manage the delivery of home health services to patients in their homes.
  • Develop and implement care plans that meet the unique needs of each patient.
  • Collaborate with multidisciplinary teams, including nurses, therapists, and social workers, to ensure seamless care transitions.
  • Provide leadership and guidance to home health staff, including RNs, LPNs, and CNAs.
  • Monitor and evaluate the quality of care provided and make recommendations for improvement.

Requirements:

  • Bachelor's Degree in Healthcare-related field.
  • Washington Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • 3 years of experience in nursing or rehab field.

Preferred Qualifications:

  • Case Management experience.
  • Supervisory experience in Home Health.
