Providence is seeking a highly skilled and experienced Home Health Clinical Manager to join our team. As a key member of our Home Health Clinical Team, you will be responsible for coordinating and managing the delivery of high-quality, patient-centered care to our patients in their homes.
Key Responsibilities:
- Coordinate and manage the delivery of home health services to patients in their homes.
- Develop and implement care plans that meet the unique needs of each patient.
- Collaborate with multidisciplinary teams, including nurses, therapists, and social workers, to ensure seamless care transitions.
- Provide leadership and guidance to home health staff, including RNs, LPNs, and CNAs.
- Monitor and evaluate the quality of care provided and make recommendations for improvement.
Requirements:
- Bachelor's Degree in Healthcare-related field.
- Washington Registered Nurse License upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- 3 years of experience in nursing or rehab field.
Preferred Qualifications:
- Case Management experience.
- Supervisory experience in Home Health.
