Responsibilities:
- Deliver direct nursing care to patients in a long-term care environment, adhering to established policies and protocols.
- Develop and monitor individualized patient care plans.
- Act as the primary coordinator for interdisciplinary patient care.

Licensure / Certification / Registration:
