Role Responsibilities:
Reporting directly to the Director of Geriatric Services, the Clinical Liaison for Geriatric Services will be responsible for facilitating communication with partner facilities, families, and patients regarding available program services and support. This role will advocate for patients by offering outreach, education, assessment, and treatment assistance to ensure access to care and collaboration among patients, families, the Care at Home team, and congregate living staff.
Key Responsibilities Include:
- Traveling to patient residences and care facilities within the Signature Health service area.
- Conducting age-appropriate face-to-face assessments for new patients in their community or home settings.
- Providing outreach to clinical and administrative staff at partner facilities to ensure effective communication and collaboration.
- Offering advocacy, education, and support to patients and families regarding aging-related issues and community resources.
- Collaborating with Care at Home clinical and administrative teams regarding specific patient needs or facility concerns.
- Facilitating community education initiatives for congregate living facilities and other organizations.
- Engaging with stakeholders in mental health and aging networks to promote program growth.
- Participating in networking and outreach business development activities.
- Maintaining patient documentation in compliance with relevant regulatory standards such as Medicare, Medicaid, OMHAS, CARF, and professional guidelines.
- Providing telehealth services as required by Signature Health.
- Completing compliance training as mandated by Signature Health.
Qualifications:
Education & Experience:
- A Master's Degree is required.
- A valid, unencumbered independent license (LISW, LPCC, or LIMFT) in the state of Ohio is required.
- 1-2 years of experience in assessment or counseling is preferred.
- 1-2 years of experience working with the aging population is preferred.
- Proficiency in Microsoft Office and Electronic Health Records is essential, along with knowledge of word processing software.
Work Environment:
- Work is typically conducted in an office or clinical environment, with additional responsibilities in home-based settings.
- While standard hours are generally maintained, flexibility for evening and extended hours may be necessary.
- Physical requirements include periods of sitting, standing, and computer work, with occasional lifting of items up to 15 pounds.
- Exposure to bloodborne pathogens may occur while performing job duties.
