Job Summary

The Inpatient Discharge Coordinator plays a vital role in ensuring a seamless transition of care for patients being discharged from our hospital. This position requires strong communication and organizational skills to coordinate with various healthcare teams and stakeholders.

Key Responsibilities
  • Participate in daily discharge planning meetings to identify patient needs and develop effective discharge plans.
  • Collaborate with inpatient and outpatient case management teams to ensure a smooth transition of care.
  • Identify and address barriers to discharge, such as social determinants of health and access to community resources.
  • Develop and maintain relationships with community healthcare providers and social services to ensure continuity of care.
  • Monitor and report on discharge metrics and quality indicators to inform process improvements.
Requirements
  • Graduate of an accredited School of Nursing with a Bachelor's degree preferred.
  • Current licensure to practice in the State of Mississippi with no encumbrances.
  • Registered with the Mississippi State Board of Nurse Examiners.
  • At least one year of direct patient care experience, with a preference for experience in case management.
Working Conditions

This role requires the ability to work in a fast-paced environment, with frequent requirements to move about the office and travel to other facilities within the SRHS service area. The work involves using repetitive motions, such as operating a computer keyboard, and requires close visual and auditory attention. The ability to work under pressure and meet deadlines is essential.
