Key Responsibilities
The Healthcare Services Manager is accountable for overseeing and coordinating clinical services, ensuring the delivery of nursing care to patients in their residences as directed by physicians and in compliance with relevant laws, regulations, and organizational policies.
Key duties include:
- Receiving referrals and assigning appropriate clinicians or therapists for timely patient evaluations.
- Coordinating the assessment of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensuring continuous assessment of patient needs and tailoring care to meet individual requirements in accordance with physician directives.
- Overseeing the development, implementation, and updates of individualized patient care plans.
- Managing communications with physicians, clinicians, patients, and referral sources to relay patient updates and new orders.
- Reviewing assessments and care plans daily, providing recommendations to clinicians as necessary.
- Coordinating care with all relevant disciplines, including physicians and medical equipment providers.
- Following up on lab results and significant changes in patient conditions to ensure proper physician notification and care plan adjustments.
- Facilitating case conferences and ensuring effective communication across disciplines, especially regarding critical findings and updates.
- Assisting in the transfer and discharge process of patients as indicated by physicians.
- Documenting and processing new orders and adjusting visit frequencies as required.
- Ensuring compliance with documentation standards and reviewing clinician visit notes for accuracy and completeness.
- Participating in quality assurance and performance improvement initiatives.
- Providing leadership and direction to clinical team members in collaboration with the clinical director.
- Engaging in ongoing professional development and completing required training.
Qualifications
- Current RN licensure in the state of practice.
- Current CPR certification.
- Valid Driver's License and access to reliable transportation.
