CarePartners is seeking a highly skilled Post Acute Transition Specialist to join our team. As a key member of our care management team, you will play a critical role in ensuring seamless transitions for patients between acute and post-acute care settings.
Key Responsibilities- Establish and maintain relationships with internal and external referral sources, including hospitals, skilled nursing facilities, and physician practices.
- Provide education and promote collaboration to optimize patient outcomes and reduce readmissions.
- Conduct screenings and assessments to ensure appropriate patient placement and facilitate smooth transitions.
- Collaborate with post-acute providers, SNFs, and physician practices to ensure quality-driven, best-practice care across the continuum.
- Maximize positive financial outcomes for designated patient populations by facilitating appropriate post-acute placement and participating in Goals of Care/Advanced Care Planning dialogue.
To be successful in this role, you will need:
- A Bachelor of Science in Nursing, or a related field, with a minimum of two years of recent experience in a health-related field.
- Strong communication and interpersonal skills, with the ability to build relationships with diverse stakeholders.
- Excellent problem-solving and analytical skills, with the ability to navigate complex healthcare systems.
- Knowledge of post-acute care services and the ability to navigate the continuum of care.
