Job Title: Transition Coordinator LP

At Vaya Health, we are seeking a highly skilled Transition Coordinator LP to join our team. As a key member of our transition team, you will play a critical role in ensuring a smooth transition for individuals from institutionalized settings to community-based services.

Key Responsibilities:

  • Develop and implement transition plans for individuals, ensuring a seamless transition to community-based services.
  • Collaborate with healthcare providers, social workers, and other stakeholders to ensure a comprehensive transition plan.
  • Conduct regular assessments and evaluations to ensure the individual's needs are being met.
  • Provide education and support to individuals and their families to ensure a successful transition.

Requirements:

  • Master's degree in a human services field with clinical licensure (LCSW, LCMHC, LPA, or LMFT).
  • Two years of post-human services bachelor's degree with accumulated experience with the population served or four years of full-time, post-non-human services bachelor's degree with accumulated experience with the population served.
  • Proficiency in Microsoft Office products and Vaya information system.
