- Reposition and Escort Residents: Safely reposition residents in bed and escort them around the community.
- Provide Toileting and Incontinent Care: Assist residents with toileting and incontinent care.
- Take Vital Signs: Monitor and record vital signs, such as blood pressure, heart rate, and pulse.
- Observe and Report: Observe and report on residents' health, escalating any problems efficiently and effectively.
- Provide Daily Reports: Detail daily reports to the Nurse or designated charge person at the end of each shift.
- Offer Emotional Support: Provide emotional support and serve as a source of comfort for residents and their loved ones.
- Recognize Abnormal Changes: Recognize abnormal changes in body functioning and report to a supervisor.
- Education and Experience: High School Diploma or equivalent, minimum 2 years' experience in a hospital or clinical setting, and valid LPN certification or State/Local equivalent.
- Skills and Abilities: Strong emotional intelligence, listening skills, and dedication to the medical field, especially working with seniors.
