About the Role

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team in a PRN/Part Time capacity. As a Nurse Practitioner, you will conduct comprehensive health assessments in the comfort of patients' homes, utilizing your expertise to identify potential health risks and provide personalized care.

Key Responsibilities:

  • Conduct thorough assessments of patients' medical history, current health status, and social environment
  • Develop and implement personalized care plans to address identified health risks and improve patient outcomes
  • Collaborate with healthcare teams to ensure seamless care coordination and communication
  • Provide education and support to patients and their families to promote healthy behaviors and disease management

Requirements:

  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills
