Position Overview
At Houston Methodist, the Advanced Physical Therapist role is integral in assessing and crafting tailored therapy plans for a diverse array of medical conditions and patient needs.

This position may focus on specialized care for intricate patient profiles, supported by advanced training and education.

As a seasoned therapist, this role involves educating patients, their families, and caregivers, while collaborating with other healthcare professionals to resolve issues and meet specific patient goals.

As a vital member of the multidisciplinary care team, the Advanced Physical Therapist provides direct patient care, ensuring adherence to safety protocols and guidelines to promote optimal patient safety, service quality, and superior patient outcomes.

This role also serves as a mentor and resource for less experienced staff, including other therapists and technicians, and may act as a clinical resource for students and clinical fellows.

The Advanced Physical Therapist assists in gathering quality metrics and analytics, contributing to the development of best practices and process enhancement initiatives as assigned.

Team Collaboration
Actively contributes to teamwork by consistently offering support and responding positively to requests for assistance. Collaborates to nurture healthy workplace relationships and provides guidance to the healthcare team, applying feedback to enhance patient outcomes.

Patient Engagement
Utilizes therapeutic communication, including active listening and teaching, to build rapport with patients, families, and the healthcare team, collaborating on care plans.

Education and Mentorship
Teaches and guides nursing and support staff, orients new team members, and assists in mentoring students during clinical rotations and clinical fellows. Promotes peer accountability to enhance department engagement scores.


Service Excellence

Patient Evaluation
Conducts thorough evaluations of patients across a wide range of diagnoses to determine their functional levels and develop treatment plans accordingly. Implements physical therapy interventions tailored to complex patient populations, ensuring cultural sensitivity and inclusivity.

Patient Education
Provides educational resources to patients and their families based on individual learning needs, consistently assessing comprehension and adapting teaching methods as necessary. Acts as a resource for clinical fellows and nursing staff in sharing best practices.

Care Coordination
Collaborates with the healthcare team to modify treatment plans, track progress, and facilitate discharge planning to achieve patient-specific goals. Advocates for patients and families to address clinical and ethical concerns using appropriate resources.


Quality and Safety

Ongoing Assessment
Regularly assesses patients, monitors progress towards treatment goals, and adjusts plans as necessary. Identifies potential complications and proactively addresses them to ensure optimal outcomes.

Oversight and Improvement
Provides oversight of physical therapy-related activities conducted by the healthcare team, adjusting plans as needed to enhance quality and maintain patient safety. Identifies opportunities for corrective actions and contributes to process improvements.


Professional Development

Documentation and Resource Management
Maintains accurate documentation of patient education and services rendered, ensuring timely entries in patient records. Efficiently utilizes resources to maximize time and equipment usage, demonstrating self-motivation in managing daily tasks.

Innovation and Growth
Initiates innovative solutions through evidence-based practices and actively participates in departmental projects and hospital committees. Identifies areas for improvement in patient care and unit operations, using evidence-based literature to support initiatives.

Qualifications

Education
Graduation from an accredited educational program recognized by the relevant credentialing body.

Experience
A minimum of two years of experience as a Physical Therapist or completion of a comparable residency program.

Licenses and Certifications
Current BLS certification and state licensure as a Physical Therapist.

Skills and Competencies
Demonstrates the necessary skills and competencies to perform assigned duties safely, with ongoing assessments of performance and capabilities.

Supplemental Requirements
Work attire includes scrubs. Employees may be required to be on-call during emergencies.
