We are seeking a highly skilled and dedicated LPN Staff Nurse to join our team at Care-Age of Brookfield. As a key member of our healthcare team, you will be responsible for providing exceptional care to our residents, ensuring their physical, emotional, and social needs are met.
Key Responsibilities:
- Deliver high-quality nursing care to residents, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Supervise and mentor Certified Nursing Assistants (CNAs) to ensure they are providing the highest level of care.
- Participate in quality improvement initiatives to enhance the overall quality of care and services provided.
Requirements:
- Current state LPN licensure and CPR certification.
- Experience in skilled nursing and rehabilitation care, with a focus on complex care.
- Strong leadership and communication skills, with the ability to work effectively in a team environment.
- Ability to work a variety of shifts, including nights and weekends.
