About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our hospice care team. As an LPN, you will play a vital role in providing direct patient care and coordinating with our interdisciplinary team to ensure the best possible care experience for our patients and their families.

Responsibilities

  • Provide direct patient care, including pain and symptom management
  • Implement nursing practice based on comprehensive assessments and care plans
  • Document patient services accurately and in a timely manner
  • Assess and educate patients and their families/caregivers
  • Participate in coordination of hospice services
  • Manage pain and symptoms effectively and evaluate patient response to treatments/medications
  • Participate in professional growth and development opportunities and quality assessment activities
  • Ensure compliance with state and federal regulations

Requirements

  • Graduate of an accredited practical or vocational nursing program with current licensure
  • 1 year of nursing experience, with 3+ years preferred
  • Experience in community health, hospice, or med/surg a plus
  • Knowledge of hospice philosophy and commitment to clinical and documentation excellence
