About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our hospice care team. As an LPN, you will play a vital role in providing direct patient care and coordinating with our interdisciplinary team to ensure the best possible outcomes for our patients and their families.

Responsibilities

  • Provide high-quality patient care, including pain and symptom management, in accordance with our comprehensive assessment and plan of care.
  • Implement nursing practices that promote patient comfort, dignity, and well-being.
  • Document patient services accurately and in a timely manner.
  • Assess and educate patients and their families/caregivers on disease management and self-care techniques.
  • Participate in the coordination of hospice services, including medication management and treatment planning.
  • Collaborate with our interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Stay up-to-date with the latest clinical practices and guidelines, and participate in ongoing education and professional development opportunities.

Requirements

  • Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation.
  • At least one year of nursing experience, with three or more years preferred.
  • Experience in community health, hospice, or med/surg settings is a plus.
  • Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
