Job Summary

Cantata Senior Living is seeking a skilled LPN Staff Nurse to join our team. As a key member of our healthcare team, you will provide high-quality care to our residents, working collaboratively with our staff to ensure their physical, emotional, and social needs are met.

Responsibilities
  • Deliver resident-centered care, prioritizing their safety and well-being.
  • Assess and develop individualized care plans, working closely with residents, families, and healthcare professionals.
  • Administer medications, treatments, and other interventions as prescribed, ensuring accurate documentation and communication.
  • Maintain accurate and up-to-date resident records, adhering to regulatory requirements and Cantata's policies.
  • Collaborate with colleagues to ensure seamless care transitions, effective communication, and a positive work environment.
  • Participate in ongoing education and training, staying current with industry developments and best practices.
  • Contribute to a culture of respect, empathy, and compassion, upholding Cantata's values and mission.
Requirements
  • Current valid certification as a Registered Nurse or Licensed Practical Nurse in the State of Illinois.
  • Current CPR certification.
  • Leadership ability and willingness to work harmoniously with and direct other personnel.
  • Effective communication and interpersonal skills, with the ability to motivate and lead individuals and groups.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Basic computer literacy and familiarity with electronic medical records (EMRs).
  • Ability to work independently and as part of a team, with a strong commitment to resident-centered care.
Working Conditions

This role requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. You must be able to lift, push, pull, and move objects weighing up to 25 pounds, and be able to sit, stand, bend, and climb stairs as needed. You will be scheduled based on operational need, and may be required to work beyond normal working hours, including weekends and holidays.
