Home Health Care Professional

We are seeking a skilled Licensed Practical Nurse to join our team at HarmonyCares, providing high-quality, coordinated health care in the home setting. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:
- Deliver intermittent skilled nursing services to patients in the home setting, adhering to their individualized plans of care.
- Conduct regular assessments and report significant findings to patients' physicians and registered nurses as needed.
- Participate in Quality Assessment and Performance Improvement (QAPI) program to ensure delivery of high-quality care.
- Apply the 4Ms framework (What Matters to the patient, Medications, Mentation, and Mobility) to provide Age-Friendly Care.
Qualifications:
- Current licensure as a licensed practical nurse in the state.
- Valid driver's license, reliable transportation, and good driving record.
- Ability to work independently and make sound nursing judgments in a field setting.
- Proficiency in Microsoft Office.
