**Job Summary**

We are seeking a highly skilled Care Manager RN to join our team in Delaware. As a Care Manager RN, you will be responsible for providing community-based care coordination and case management services to our clients.

About the Role:

This is a full-time position that requires assessing, planning, implementing, and evaluating care for individuals with chronic and complex health needs. You will work closely with healthcare teams to ensure seamless transitions and optimal outcomes for our clients.

Key Responsibilities:

  • Conduct face-to-face assessments and telephonic contact with clients to understand their needs and develop personalized care plans.
  • Collaborate with healthcare providers, family members, and caregivers to ensure continuity of care.
  • Develop and implement service plans that address the physical, behavioral, and social needs of clients.
  • Coordinate care across the continuum of services, including hospital to home and nursing facility to community settings.
  • Educate clients and their families on health care options, resources, and benefits.

Requirements:

To be successful in this role, you must have:

  • A valid Registered Nurse (RN) license in the state of Delaware.
  • At least 2 years of experience in case management and discharge planning.
  • Strong communication and interpersonal skills.
  • The ability to work in a fast-paced environment and prioritize multiple tasks.

Salary:** $65,000 - $85,000 per year, depending on experience.
