About the Role

The Case Manager, RN provides community-based care coordination and management for members with chronic and complex healthcare needs in Delaware. This role requires a current RN license in Delaware and involves traveling to various settings to assess member needs and facilitate transitions in care.

Key Responsibilities
  • Conducting assessments to identify member needs and goals
  • Developing and implementing individualized care plans to ensure optimal care
  • C coordinate services and resources to support member needs
  • Educating members on available resources and health options
Required Skills and Qualifications
  • Current RN license in Delaware
  • 2+ years of experience in case management and discharge planning
  • Experience working with geriatric special needs, behavioral health, and home health populations
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
