Job Summary

We are seeking an experienced Healthcare Navigator Professional, RN to join our team. As a key member of our care coordination team, you will be responsible for providing comprehensive care navigation services to clients with complex health needs.

About the Role

The ideal candidate will have strong clinical expertise in community-based care and experience in case management. You will work closely with our interdisciplinary team to assess client needs, develop personalized care plans, and facilitate transitions between care settings.

Responsibilities
  • Conduct thorough assessments of client needs, identifying social determinants and healthcare gaps.
  • Develop and implement comprehensive care plans that address client goals and preferences.
  • Facilitate smooth transitions between care settings, ensuring seamless continuity of care.
  • Collaborate with healthcare providers, family members, and caregivers to ensure coordinated care.
  • Maintain accurate records and reports, documenting client progress and care outcomes.
Requirements

To be successful in this role, you will need:

  • A current Registered Nurse license in the state of Delaware.
  • At least 2 years of experience in case management and discharge planning, preferably in a community-based setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong analytical and problem-solving skills, with the ability to think critically and creatively.
  • Ability to work independently and as part of a team, prioritizing tasks and managing competing demands.
  • Proficiency in electronic documentation systems and Microsoft Office applications.
