Job Summary

We are seeking a motivated and experienced Registered Nurse to join our team as a Healthcare Navigator in Delaware. In this role, you will be responsible for coordinating and managing community-based care for clients with complex healthcare and social service needs.

Key Responsibilities:
  • Conduct assessments and develop care plans to meet the unique needs of each client
  • Facilitate transitions between care settings, ensuring seamless coordination and continuity of care
  • Educate clients and caregivers on healthcare options, benefits, and resources available to them
  • Collaborate with healthcare teams, including PCPs, OTs, BH specialists, and providers, to ensure comprehensive care planning
  • Travel to members' homes, nursing facilities, and other community-based settings to provide face-to-face support and guidance
Requirements:
  • Current RN licensure in the state of Delaware
  • 2+ years of experience in case management and discharge planning, with a focus on geriatric special needs and behavioral health
  • Proven ability to work effectively with diverse populations, including those with HIV/AIDS, developmental disabilities, and chronic conditions
  • Strong communication and interpersonal skills, with the ability to build trust with clients and their families
  • Familiarity with electronic documentation systems and proficiency in Microsoft Office
