The ideal candidate will be comfortable traveling to various locations to meet clients and will be responsible for transporting clients using company vehicles. Understanding trauma-informed care is crucial, as we apply this framework in our interactions with clients. Additionally, a solid grasp of HIPAA regulations and their relevance to case management is essential.
Key Responsibilities:
- Evaluate the unique needs of clients
- Develop personalized service plans with direct client input in accordance with CCSS requirements
- Collaborate with other AHCH programs to enhance client care
- Connect clients with appropriate programs and agencies for comprehensive community services
- Engage in client chart audits for peer review
- Deliver direct services to clients, with 60% of interactions occurring through home and community visits
- Conduct all duties in a trauma-informed and client-centered manner
- Maintain accurate documentation of all client interactions in the Electronic Health Record (EHR) as per AHCH procedures and billing standards
- Participate in program-specific meetings and training, including clinical supervision with the Behavioral Health Clinical Director
- Attend organizational meetings and training sessions
- Adhere to all AHCH policies and procedures
Minimum Qualifications:
- Ability to work in a client-centered, trauma-informed, and harm reduction manner to support clients in achieving their goals
- Bachelor's degree in a human services field or equivalent experience
- Two years of experience working with underserved populations
- Valid driver's license and a clean driving record
Preferred Qualifications:
- CCSS certification
- Experience with trauma-informed care and harm reduction models
- Bilingual or Spanish-speaking skills
- Previous experience working with individuals with serious mental illness/chronic mental illness
