- Immunization Certification: Possess immunization certification through an accredited organization.
- No Felony Charges: Not have any pending felony charges or convictions for criminal offenses involving controlled substances.
- Education: Hold a Bachelor of Science in Pharmacy or Pharm. D. degree.
