Job Summary

We are seeking a skilled Registered Nurse to join our Hospice team as an Intake Coordinator. In this role, you will play a vital part in coordinating referrals from physicians, hospitals, and other community referral sources.

Key Responsibilities
  • Coordinate referrals and ensure seamless transitions for patients and families
  • Collaborate with healthcare teams to provide comprehensive care and support
  • Utilize critical thinking skills to make informed decisions regarding patient care
  • Foster a learning environment for patients, families, and healthcare teams
Requirements
  • Bachelor's degree in Nursing or equivalent experience
  • Minimum of two years of Medicare Certified Home Health and/or Hospice nursing experience
  • Advanced degree in Nursing preferred
