Job Overview

The Hospice Care Coordinator plays a critical role in our team, working closely with patients, families, and healthcare professionals to ensure seamless care delivery. This position requires strong communication skills, empathy, and a passion for delivering exceptional patient experiences.

Responsibilities
  • Care Coordination: Develop and implement individualized plans of care that meet the unique needs of each patient, incorporating input from the Interdisciplinary Group (IDG) and other healthcare professionals.
  • Patient Education: Educate patients, families, and caregivers about disease processes, palliative interventions, and safety practices, promoting informed decision-making and empowerment.
  • Bereavement Support: Provide bereavement interventions to families and caregivers, offering emotional support and resources during times of need.
  • Communication: Effectively communicate with IDG members, physicians, staff, patients, families, and supervisors, utilizing clear and concise language and approaches.
Requirements
  • Education: Current licensure as an RN in the state where the employee will be working.
  • Experience: Minimum one year of in-patient nursing or hospice experience for external candidates; internal applicants require minimum 6-months of LPN experience with hospice/home health within an affiliate.
  • Skills: Previous experience working with an EMR/EHR system, mobile driver's license, reliable transportation, ability to use equipment with visual and auditory mechanisms, effective communication in English (verbal and written), and flexibility to meet the needs of Hospice House patients.
