MDS Coordinator RN

We are seeking a skilled and compassionate MDS Coordinator RN to join our team at Beaverton Rehab & Specialty Care LLC. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our residents.
Responsibilities- Develop and implement individualized care plans that meet the unique needs of each resident.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Conduct thorough assessments and reviews to identify areas for improvement and implement changes as needed.
- Manage and maintain accurate and up-to-date resident records, including medical histories and treatment plans.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current RN licensure in the state of Oregon.
- Experience in long-term care or gerontology preferred.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Proficiency in electronic medical records and computer documentation systems.
