About the Role

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Per Diem team at Elderplace Beaverton. As a Per Diem RN, you will provide care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.

Key Responsibilities
  • Provide care management and care coordination services to patients and their families.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and services.
  • Travel between multiple locations to provide care and services to patients.
Requirements
  • Oregon Registered Nurse License
  • National Provider BLS - American Heart Association within 30 days of hire
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • 1 year Home health RN experience
