About the Job

Rochester Center for Rehabilitation and Nursing is seeking a skilled Licensed Practical Nurse (LPN) to join our team in our Skilled Nursing Facility located in Rochester, NY.

Job Summary

We are looking for a dedicated and compassionate LPN to provide high-quality patient care and support our residents in their rehabilitation and recovery journey. As an LPN at Rochester Center, you will have the opportunity to work in a dynamic and supportive environment, collaborating with a multidisciplinary team to deliver exceptional patient outcomes.

Responsibilities
  • Record and maintain accurate and detailed health information for residents, including vital signs and temperature.
  • Administer medications and injections as prescribed by physicians and in accordance with facility policies.
  • Treat and dress wounds and bedsores as needed, ensuring proper wound care and prevention.
  • Supervise and assist Certified Nursing Assistants (CNAs) in providing resident care, ensuring that tasks are completed efficiently and effectively.
  • Assist residents with daily living activities, such as bathing, dressing, and personal hygiene.
  • Monitor residents' food and liquid intake and output, ensuring that their nutritional needs are met.
Requirements
  • Must be a licensed LPN in the state of New York.
  • Must be in good standing with the New York State Board of Nursing.
  • Must be able to work effectively as a team member, communicating clearly and respectfully with colleagues and residents.
