Job Summary

We are seeking a skilled RN MCH Per Visit Specialist to join our team at Danbury Health Systems. This is a per diem opportunity that requires flexibility and a commitment to delivering high-quality patient care.

The ideal candidate will have zero to six months of experience in a similar role and possess strong nursing skills, including assessments, goal-directed planning, and implementation of care. A BSN degree and nursing certification are highly desirable.

This position offers a competitive salary range of $32.64 to $65.01 per hour, depending on qualifications and experience. The working schedule is varied, with standard hours of 1.00 and an FTE of 0.010000.

Responsibilities:

  • Perform ongoing and systematic assessments of patients' physiologic, psychological, and cognitive status.
  • Formulate a goal-directed plan of care prioritized based on determined nursing diagnosis and patient outcomes.
  • Implement care in a knowledgeable, skillful, consistent, and continuous manner.
  • Establish priorities of patient care based on essential patient needs and available resources.
  • Evaluate the effectiveness of care given and patient progression toward outcomes.
  • Identify patients' learning needs and implement appropriate measures to meet them.
  • Coordinate interdisciplinary functions.
  • Fulfill all compliance responsibilities related to the position.
  • Perform other duties as assigned.
