Care Transition Coordinator RN

At Universal Health Services, we are seeking a highly skilled Care Transition Coordinator to join our team. As a Care Transition Coordinator, you will play a critical role in ensuring the seamless transition of patients from hospital to home or other care settings.
Key Responsibilities:
- Assess the needs of patients and coordinate resources to promote optimal health benefits and outcomes.
- Actively participate in daily rounds with nursing and medical staff to determine high readmission possibilities and follow through with patients, primary care physicians, insurers, and other services to ensure a safe discharge.
- Address and resolve system issues impeding diagnostic or treatment progress with assigned populations.
- Monitor patient progress and intervene as necessary to ensure the plan of care and services provided are patient-focused, high-quality, and cost-effective.
- Provide education as needed to staff, physicians, and patients for transitional care planning needs.
- Seek consultation from appropriate disciplines for ongoing care planning.
- Refer cases and issues to appropriate personnel in compliance with department procedures and follow up as indicated.
- Document relevant care transition planning information in the medical record according to department standards.
Requirements:
- Graduation from an accredited nursing program.
- License by State of SC, compact state.
- Minimum of 3 years Utilization Review/Case Management experience preferred.
- Recent working knowledge of Interqual criteria and Milliman Care Guidelines preferred.
- Minimum of 3 years in clinical med-surg nursing practice within a hospital setting.
