Care Transition Coordinator Job Description

At Universal Health Services, we are seeking a highly skilled Care Transition Coordinator to join our team. As a Care Transition Coordinator, you will play a critical role in ensuring the smooth transition of Medicare patients from hospital to home, promoting optimal health outcomes and reducing readmissions.

Key Responsibilities:

  • Assess patient needs and coordinate resources to promote optimal health benefits and outcomes
  • Actively participate in daily rounds with nursing and medical staff to identify high readmission possibilities
  • Develop and implement individualized plans of care to ensure patient-focused, high-quality, and cost-effective care
  • Provide education to staff, physicians, and patients on transitional care planning needs
  • Collaborate with multidisciplinary healthcare teams to ensure seamless care transitions

Requirements:

  • Graduation from an accredited nursing program
  • License by State of SC, compact state
  • Minimum of 3 years Utilization Review/Case Management experience preferred
