About the Position
">We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health and Hospice Nurse. As a key member of our healthcare team, you will provide personalized patient care in the comfort of patients' homes or at our inpatient hospice facility.
Job Description
This is a dynamic and challenging role that requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care. The ideal candidate will possess a strong work ethic, be able to work independently, and demonstrate a commitment to providing exceptional patient-centered care.
Key Responsibilities
- Conduct thorough assessments of patients' physical and emotional needs, identifying opportunities for improvement and implementing effective interventions.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Communicate effectively with patients, families, and caregivers, providing education and support to empower informed decision-making.
- Maintain accurate and timely documentation of patient care, adhering to regulatory requirements and agency policies.
Required Qualifications
- Degree in Nursing from an accredited institution.
- Current North Carolina RN licensure or compact RN licensure in participating state.
- Valid driver's license and current automobile insurance coverage.
- Prior experience in home health, hospice, or infusion preferred.
