Job Summary

We are seeking a highly skilled and compassionate Oncology Care Coordinator to join our team at Hackensack Meridian Health. As a key member of our cancer care team, you will play a vital role in ensuring that our patients receive the highest level of care and support throughout their journey.

Key Responsibilities
  • Coordinate and Navigate Outpatient Care: Identify new patients during the pre-diagnosis journey and offer navigation services from the beginning of the patient's diagnosis journey to survivorship or end of life.
  • Build and Maintain Professional Relationships: Establish and maintain professional relationships with medical oncologists, radiation oncologists, surgeons, and other healthcare professionals involved in patient care.
  • Communicate Effectively: Establish communication with physicians, physician extenders, and their office staff to outline navigation programs and available resources.
  • Facilitate Access to Services: Facilitate access to services, track the flow of patients through diagnostic and treatment services, and document assessment and follow-up notes in the patient's medical record.
  • Provide Ongoing Assessment and Support: Provide ongoing assessment of patient needs, including educational, financial, psychosocial, nutritional, and clinical needs, and connect patients and families to available resources within the health system and in the community.
  • Collaborate with Research Department: Work collaboratively with the research department to ensure that patients have access to the latest treatment options and clinical trials.
  • Participate in Performance Improvement Activities: Participate in department-specific committees and represent patient navigation services at committee meetings to ensure that all requirements of regulatory standards for patient care are met.
  • Represent the Cancer Program: Represent the Cancer Program's services to the community, collaborate with marketing for brochures and other marketing materials, and identify and link patients to available community partners and services.
Requirements
  • Education: Bachelor of Science in Nursing (BSN)
  • Experience: Minimum of two years of experience in nursing
  • Skills: Excellent written and verbal communication skills, proficient computer skills, and ability to work effectively in a fast-paced environment
  • Certifications: NJ State Professional Registered Nurse License, Oncology Certified Nurse (preferred)
