RN Case Manager

The RN Case Manager is a vital member of our team at Compassus, responsible for delivering exceptional patient care and promoting a culture of compassion and excellence. As a RN Case Manager, you will be the primary point of contact for patients and families, providing guidance and support throughout the care journey.
Key Responsibilities
• Develop and implement personalized care plans that meet the unique needs of each patient
• Collaborate with healthcare professionals to ensure seamless care coordination
• Provide education and support to patients and families on disease management and self-care
• Conduct thorough assessments to identify patient needs and develop effective care strategies
• Communicate effectively with patients, families, and healthcare teams to ensure timely and effective care
Requirements
• Current RN licensure and a minimum of 2 years of experience in home health or hospice care
• Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
• Ability to work independently and as part of a team, with a focus on delivering exceptional patient care
• Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions
• Proficiency in electronic medical records and other healthcare software
