We are seeking a compassionate and dedicated individual to join our team as a Case Manager Intern. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of patients and clients.

About the Role

Under general supervision, you will provide clinical case management and/or specialized supportive and supplemental services to clients to aid them in improving their social, economic, cultural or educational status or to assist them in restoring their capacity for social functioning.

Responsibilities
  • Provide direct service to clients, either in the office or in the field
  • Review and monitor services, reassess needs, and initiate services which are responsive to the needs of clients
  • Assist clients in developing formal and informal community supports
  • Provide advocacy on behalf of clients
  • Prepare and complete all required case recordings, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner
Requirements
  • Combination of education and experience normally represented or working to obtain a Bachelor's Degree
  • Competent oral, written and interpersonal communication skills
  • Computer fluency with ability to complete documentation within an electronic medical record
  • Well organized, detail oriented and ability to maintain confidentiality
  • Ability to work with diverse client population, community organizations as well as staff at all levels
