Career Opportunity:

CDS Life Transitions is seeking a highly skilled Speech Language Pathologist to join our team in providing direct Speech Therapy services to participants through the Article 16 clinics. The ideal candidate will possess a Certificate of Clinical Competence (CCC) and be licensed and registered as a Speech-Language Pathologist by the New York State Department of Education.

Responsibilities:

Provide physician ordered speech therapy services.
Ensure physician orders are renewed, per OPWDD Article 16 requirements.
Complete clinical documentation, including initial assessments, daily notes, renewal orders, progress reports, and discharge summaries.
Identify patient short/long term goals and reflect progress in charting.
Complete communication and swallowing evaluations and develop appropriate programs.
Help patients work towards accomplishing their treatment goals using therapeutic devices.
Attend interdisciplinary staff meetings and supervise ST students/new grads as assigned.

Indirect Clinical Services:
Complete observations of diet consistency and recommend equipment and assistive devices as needed.
Update/develop Individualized Plans of Care or Conditions based on individual needs.
Provide training/supervision to direct care staff regarding treatment plans and consultation to other team members as needed.
Participate in committees and program development, as indicated.

Requirements:
Possess or be eligible for a Certificate of Clinical Competence (CCC).
Licensed and registered as a Speech-Language Pathologist by the New York State Department of Education, or eligible for licensure by virtue of holding a Certificate of Clinical Competence (CCC) issued by the American Speech-Language-Hearing Association (ASHA).
Reliable transportation and ability to work independently and motivate others.
Ability to communicate effectively, both orally and in writing, and lift at least 50 lbs. dead weight.
