This health system encompasses three hospitals, an outpatient surgical center, a regional cancer center, rehabilitation facilities, two long-term care residences, and comprehensive home care and hospice services, alongside 34 affiliated physician practice locations. We deliver both inpatient and outpatient medical services across Southwest Michigan.

Role Overview

The RN, adhering to the ANA scope of practice, employs the nursing process framework to deliver professional nursing care, integrating patient-family centered care principles to assess, plan, implement, evaluate, and discharge patients. The RN ensures high-quality patient care under the guidance of the Nurse Leader, in alignment with Corewell Health policies and procedures, and acknowledges the influence of nursing care and patient/family partnerships on health outcomes. The RN collaborates with patients and families to incorporate their input and objectives into the care plan, developing educational strategies for them and accurately documenting this in the health record.

Key Responsibilities

  • Conducts thorough reviews of nursing charts and performs evaluations as necessary to complete the Minimum Data Set (MDS) in compliance with state regulations and Medicare A requirements. Establishes weekly MDS schedules and Care Conference calendars, making adjustments as required.
  • Ensures the timely completion of the Resident Assessment Instrument (RAI) by the interdisciplinary team, collaborating on the RAI and care planning process while identifying significant changes in resident conditions.
  • Participates in weekly team conferences and daily clinical management meetings, overseeing Medicare A coverage and ensuring the completion of Medicare certifications and re-certifications.
  • Conducts audits and quality reviews as assigned by the Manager, monitoring the accuracy of MDS items completed by other departments and providing necessary information for billing.
  • Stays informed on evolving reforms related to MDS and Prospective Payment System (PPS), supplying relevant information for Access Management as needed.

Qualifications

  • Associate's degree in Nursing required upon hire.
  • Bachelor's degree in Nursing required within five years.
  • Graduates from non-accredited nursing schools must have at least one year of acute care experience or two years in a licensed healthcare role and must have successfully passed the NCLEX licensure examination.
  • MI Registered Nurse (RN) license required upon hire.
  • AHA or ARC Basic Life Support (BLS) certification required within 90 days.
  • CRT-Nurse Assessment Coordinator, Registered (RNAC) certification from AANAC required within one year.
