Pine Ridge is a well-established 111-bed Skilled Nursing Facility, certified by Medicare and Medicaid, featuring 33 private rooms and 39 semi-private rooms. Nestled within the scenic Hansen Care Park, our facility offers spacious shower and restroom amenities, beauty salon and barber shop services, walking paths, and advanced rehabilitation areas for individuals requiring physical, occupational, or speech therapy, along with cozy relaxation spots throughout the premises. We cater to residents needing a range of medical services, including Short-term Rehabilitation, Long-term Care, 24-Hour Skilled Nursing, Hospice and Respite Care, Restorative Care, and Certified Wound Care.
About Corewell Health South – Lakeland Hospitals
This health system encompasses three hospitals, an outpatient surgery center, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 affiliated physician practice locations. We deliver both inpatient and outpatient medical services across Southwest Michigan.
Role Overview
The RN, adhering to the ANA scope of practice, employs the nursing process framework to deliver professional nursing care by integrating patient-family centered care principles to assess, plan, implement, evaluate, and discharge. The RN ensures high-quality patient care under the guidance of the Nurse Leader, in accordance with Corewell Health policies and procedures, while recognizing the influence of nursing care and patient/family collaboration on patient outcomes. The RN collaborates with patients and families to incorporate their input and objectives into the care plan, developing educational strategies for patient/family learning and documenting appropriately in health records.
Key Responsibilities
- Conducts thorough reviews of nursing charts on resident information and performs evaluations as necessary to complete the Minimum Data Set (MDS) in compliance with state regulations and Medicare A requirements. Establishes weekly MDS schedules and Care Conference calendars, making adjustments as needed.
- Ensures timely completion of the Resident Assessment Instrument (RAI) by the interdisciplinary team, collaborating on the RAI and care planning process, and identifying significant changes in resident status.
- Participates in weekly team conferences and daily clinical management meetings. Monitors Medicare A coverage and the completion of Medicare certifications/re-certifications, informing the Social Worker of the last day of Medicare A coverage.
- Performs audits and quality review tasks as assigned by the Manager. Monitors the accuracy of MDS items completed by other departments and provides necessary information to billing.
- Stays informed on evolving reforms related to MDS and Prospective Payment System (PPS). Supplies PPS information as required for Access Management.

Qualifications
- Associate's degree in Nursing required upon hire.
- Bachelor's degree in Nursing required within five years.
- Graduates from non-accredited nursing schools must have at least one year of acute care experience or two years in a licensed healthcare role and must have successfully passed the NCLEX licensure examination.
- Michigan Registered Nurse (RN) license required upon hire.
- AHA or ARC Basic Life Support (BLS) certification required within 90 days.
- CRT-Nurse Assessment Coordinator, Registered (RNAC) certification from AANAC required within one year.
